MonaLisa Touch laser treatment has often resulted in improvement of common menopausal symptoms like vaginal dryness, painful sexual intimacy and vaginal/vulvar itching that are due to the loss of estrogen and collagen.  This loss occurs when estrogen levels are low as experienced in natural menopause or in surgical removal of the ovaries.  Many women are experiencing improvements in painful voiding and leaking of urine after having the MonaLisa treatments.  The MonaLisa Touch treatment provides a comfortable, safe non-hormonal option for women who either do not want to use estrogen, have tried estrogen products without success or are unable to use estrogen to for medical reasons such as a history of breast cancer, or are struggling with the financial costs of long term hormonal therapy.  Many women also find they no longer need to use vaginal lubricants or moisturizers after this laser therapy.  MonaLisa Touch treatments are done in a relaxed office setting.  Typically 3 brief treatments are required and result in very little interference with daily activities or downtime.

Vaginal dryness is primarily caused by the loss of estrogen and collagen over a period of time that usually starts after menopause but may begin way before.  Estrogen deficiency results in a decline in collagen in vaginal and vulvar tissues.  Collagen is partly responsible for the plumpness or thickness of the skin and vaginal lining as well as natural vaginal moisture production.  As collagen is depleted the vagina loses its elasticity, the lining thins, and moisture decreases.  This is when many women notice pain with intercourse, dryness and irritation.  Often there is also an increase in painful urination, urinary frequency, incontinence and pelvic pain.

Clinical studies have shown that the MonaLisa Touch laser treatment may improve vaginal dryness and painful intercourse by enhancing collagen formation.  Many women experience complete relief in vaginal dryness and report alleviation of discomfort felt during intercourse.

The MonaLisa Touch is a unique laser system that delivers energy to the vaginal lining and vulva at safe levels to stimulate tissue changes which result in the development of new collagen.  The laser energy starts a cascading effect of cellular changes within the very top layer of the vaginal lining.  Over the next few weeks and months, as collagen develops, the vaginal lining thickens, circulation to the tissue is enhanced and moisture is improved.

Most patients feel a deep warmth and tingling sensation intermittently throughout the treatment which is generally well-tolerated.

The system is Monopolar. This means that there is an active electrode handpiece (HP) and a neutral pad (antenna) to complete the circuit.
There is also a Bipolar mode, which is only used for surgical applications.

Pellevé FDA Clearance: Full face non-ablative treatment of mild to moderate facial wrinkles and rhytids for all skin photo types.
This means Pellevé is cleared for the treatment of the full face, which includes the face and and under the chin. Pellevé can be
used on any skin type because RF energy is not absorbed by melanin. 

The handpieces are simply “transmitters” of energy and do not get hot. Radiofrequency energy uses the tissue’s resistance within the various layers of the skin to transform RF energy into thermal energy. That resistance (or impedance) is what creates the deep heat. Remember, RF energy has no absorption in melanin so the Pellevé is safe to use on all skin types. Radiofrequency is different than a laser or IPL, these devices emit intense light that can damage surrounding tissues and can be absorbed by epidermal melanin. The delivery of RF energy causes inflammation deep in the dermis which triggers a wound healing response. The wound healing response promotes the production and deposition of new collagen and remodels damaged collagen. This results in “plumping” of the skin which minimizes the appearance of wrinkles.

Usually a full facial treatment takes about 45-60 minutes.

The 10mm and 20mm are used most frequently.

The handpieces have a 3 hour life span. We call them “Limited Life Reusable” as they can be used for multiple clients. There are 2 LED lights on the handpieces: 1. At the cone, which lights up when the electrode is depressed and energy is flowing. 2. At the base, which shows how much time is left on the handpiece.

Wipe the handpieces with 70% Isopropyl Alcohol. DO NOT submerge the handpieces in any liquid.

Immediate Gratification Effect – the client will see an immediate result post treatment. This is due to the initial swelling
(edema) and also related to the plumping of the connective tissue. This result may last for 1-3 days.
Long Term Effect – Clients will begin to see incremental results in a few weeks; the collagen remodeling process continues for about 3-6 months after the last treatment.
Maintenance – After the recommended series of treatments are completed, clients usually have a maintenance treatment every 6 months, beginning 6 months after the last treatment.

Adhesive neutral pad must be used for all Pellevé treatments. It is very important that the pad is in full contact with the skin.
When treating the face, the pad is usually placed on the back between the client’s shoulder blades.

If a client has a titanium implant in their shoulder or cervical spine, the neutral pad may be placed on the chest or abdomen to change the path of energy. RF will always follow the path of least resistance so neutral pad placement is an important consideration. If a client has titanium facial hardware or a titanium plate in their head, they cannot be treated. 

There have not been any reported issues with dental implants and the Pellevé treatment. For additional protection place
wet 4X4 gauze under their lips/over their teeth.

Clients that have the following should not have Pellevé treatments:

• Pacemaker
• Imbedded Defibrillator
• Any implanted device that gives or receives an electronic signal.

Plastic corneal eye shields should be inserted into a client’s eyes if you are treating the eyelid down to the lash line or for any surgical applications in the orbital area. The shields are not necessary if you are not treating the eyelid to the lash line. Here’s a tip: retract the brow so that the treatment is given over the orbital rim with the handpiece pointed away from the globe of the eye.

All metal jewelry in the treatment area should be removed, this includes earrings, nose rings/studs, and necklaces, also metal hairbands need to be removed. Tongue piercings do not have to be removed.

We recommend that you administer 3-5 treatments depending on client’s age, gender, skin condition and tissue thickness. Some older clients (60+) may choose to have 5 or more treatments.

Your client can expect a deep warming sensation which should not be painful. Frequently check in with your client thorough out the treatment to be sure they are comfortable. Topical anesthetics should not be used for the Pellevé treatment.

After Treatment there will be some erythema (redness) and edema (swelling). The erythema usually resolves by the time the client leaves the office and the edema plumps the skin.
There is no downtime and the client can apply makeup immediately post-treatment. The client can resume their usual skin care regimen.

If any of these have been already given, please allow 2 weeks before a Pellevé treatment. But they can be given any time after
the Pellevé treatment, even at the same appointment, provided the neurotoxins or fillers are given after the treatment.

Pellevé can be paired with other aesthetic treatments, as long as Pellevé is done first.

For example: Pellevé/CO2 fractional, Pellevé/IPL, Pellevé/Peels. Many plastic surgeons treat clients with Pellevé, then perform a face lift, and have client return in 3 months to resume Pellevé treatments for 3 months.
